Practice Information

Twenty-seven of the Michigan IPIP practices received Patient Center Medical Home (PCMH) designation by Blue Cross Blue Shield of Michigan (BCBSM) in June 2010. In addition, all thirty-three practices, involving 206 physicians, have begun better understanding the diabetic or asthma patient population within their practice. Twenty-five practices have started actively managing the associated care to these protocols using either EHR/EMRs or Registries. The journey into transforming these practices has started & although the project and its funding has officially ended, practices continue to participate in monthly calls. In addition, several still keep in contact with their coaches.

IPIP has started transforming Michigan into a healthier state. Today, leadership is exploring means for this initial effort to transition into a broader program.
